Put a villain in the background. Over the course of smaller adventures, have the heroes find out small things. Have that villain run a plot in the background that the heroes get pieces of, eventually having the big showdown.

Depends on at what point in her career. There was a time when Jen couldn't change out of She-Hulk even if she wanted to. There was a point when Tony Stark made it so that she couldn't change into She-Hulk if she wanted to. Generally, I'd just make her a multiform/not-quite-instant change, with only her physical attributes changing.

Malone was dead, which was why Batman could get away with the act. I thought it was Spoiler initiating Batman's Wargames project that ruined Malone's cover for him.

Neither does Superman. That version died after the Crisis and he was rebooted in Byrne's Man of Steel. I don't even think the gnu52 Sooperman does all that.

Team? I usually end up with the JLA type of teams (groups of individuals) rather than the X-Men type of team, where they actually work as a team.
