I myself, on several occasions, have ordered straight off the value menu--i.e., "I'll have the number 3 with a diet, please"--and had it screwed up, in one case because they handed me someone else's order. Again, I have very low expectations of minimum wage employees working in fast food... but when you're hungry and in a rush, it can really ruin your whole day. The fact that mayonaise is apparently the default condiment means that exactly that is the case. It is about as special as "any burger, without the piece of salad". I would not expect them to get that done either. When asked if I want "Ketchup or Mayonaise?" to my fries, I just say "neither". I tried shortcutting that (mentioning I want neither earlier or just frontloading the order), but that seemed to only cause more chaos. So in the end I just wait for them to work of thier questionary, Say "neitehr" at the right point. And it works out for everyone involved. The pride of lions were not exactly, you know, expecting human visitors, so they immediately attacked the intruders. The attack was so instantaneous that five of the ten men were killed, three severely injured, and two lightly injured. The animals were mostly unhurt. And no they don't ask, so you have to tell them specifically no mayonnaise. Then, they look at you with extreme confusion like you just gave them the answer to a difficult calculus problem. ANd then put the mayonnaise on the burger. To be fair, every other place I have little to no trouble with, and I long since stopped going to McDonald's. By definition the person taking your order either didn't grow up speaking English or just flunked out of high school.
